As a General note, Apple uses a contact cement in its assembly processes that has very good adhesion characteristic's, I find that once you can work your way under the piece you can cut away at the cement freeing the part from the adhesive.
Before attempting this please wear eye protection, small fragments of this glass can cause sever eye damage in the event the glass shatters.
Before attempting any repair be sure to ground yourself with an ESD(electro static discharge) strap. This will prevent static electricity from damaging the electronic components within the phone.

Tools and Parts required:
New LCD/Glass screen
Pry Tools
Phillips Screw driver
Anti static strap
Removing the Rear Cover:
- Using the pry tools work a space between the frame and the rear cover.
- Once a space is created insert the pry tool to lift the rear cover from the frame
- Continue applying force around the edge to completely lift the cover
Battery Removal:
- Use the pry tool to lift the battery
- This will take a few minutes as there is a lot of contact cement securing the battery
- Start from the side opposite the power wires by lifting the battery with the pry tool
- As the battery lifts gently slide the pry tool under the battery
- Use caution as there are ribbon cables underneath that could be damaged
Main Board:
- Lift the black protective tape that is on the main board to reveal two additional screws
- Save the tape is it secures the ribbon cable and you will attach it during assembly
- Remove the screws that attach the main board to the frame
- Remove the lower board by removing the 2 Phillips screws
- Using the pry tool lift the board from the frame
- You must also lift the small board that is cemented to the frame
- This can be done with the pry tool as seen in the video
- Before lifting the board disconnect the cable that is attached to the board
- Lift the board and ribbon cable as a whole
- The bottom half of the main board is secured with contact cement to the frame
- Using the pry tool lift the bottom half from the frame
- The board may bend slightly, be patient and slowly lift the board minimizing bending
- Once the board is removed it can be flattened to fit properly during installation
LCD Removal:
- Now the main board is out of the way you can remove the LCD
- Start by removing the yellow protective tape covering the 16 Phillips screws
- Once the tape is removed you can remove the 16 screw and store them for later installation
- Lift the LCD from the frame and store it in a safe spot for installation
It is likely you will can reuse the adhesive that holds the digitizer in place. In the event you have scraped it all away, I recommend using a Double-Sided-Sticky tape to adhere the glass to the phone.
