As a General note, Apple uses a contact cement in its assembly processes that has very good adhesion characteristic's, I find that once you can work your way under the piece you can cut away at the cement freeing the part from the adhesive.
Before attempting this please wear eye protection, small fragments of this glass can cause sever eye damage in the event the glass shatters.

Tools and Parts required:
New LCD/Glass screen
Pry Tools
Phillips Screw driver
Anti static strap

Removing the Glass from the frame:

  1. Find a spot where the glass is damaged and using a pry tool lift the glass from the frame
  2. Once the pry tool is under the glass slide the pry tool under the glass along the edge to release the contact cement
  3. The glass is secured at the top, the bottom and along the edge
  4. Once the majority of the glass has been removed you will need to lift the remaining pieces off carefully.
  5. I do not recommend scraping these pieces
  6. Leave the contact cement in place to adhere the new glass to the frame
  7. The home button is held in place by the glass
  8. Once the glass is removed the button will be free to be removed
  9. Be sure to save the home button as it will be used during installation
  10. Take a few minutes to thoroughly clean all the fine pieces of glass from the frame as any small piece of glass could later cause the new screen to crack
It is likely you will can reuse the adhesive that holds the digitizer in place. In the event you have scraped it all away, I recommend using a Double-Sided-Sticky tape to adhere the glass to the phone.
